From a medical/science standpoint, what are the limiting factors that make it difficult to produce pharmaceutical drugs that do not have negative side effects?

From a famous poem by Elizabeth Bishop.;....Even losing you,(the joking voice, a gesture I love) I shan't have lied. It's evident the art of losing 's not too hard to master though it may look like(Write it!) like disaster.
The question is, what's the difference between " I shall never lie." and"I shan't have lied."??
