With email and website passwords, are successful "brute force" attacks still common, where an automated bot tries thousands and thousands of passwords on the same user account until one works?

Can you help me understand how to approach a couple issues of server security. (1) I want to run things in 'silos', so that if someone from the web has hacked and has code level access to example.com/blog they can't query the db of example.com/app (2) If they do get access to a user database, how do I make it harder for them to figure out passwords (beyond just storing them as md5)?

Is Randall Munroe right about passwords in today's
xkcd? Are very long (all lower case) plain text passwords more secure than short ones using a mix of numerals and upper/lower case text?

Up to now, I have used Apple's Keychain program to manage my passwords. I recently got an iPad, and now I have lots of login passwords that I don't have memorized and can't access from my iPad browser. I typically use Safari, and have my preferences set to store all of my passwords and user names in my Keychain.app. However when it starts acting up and giving me the spinning beach ball, I use Camino (v. 1.0.3.) When I try to log in to a website, it asks me for my user name and password - do I really have to fill in ALL those passwords and user names all over again? In other words, is there any way I can get Camino to access the passwords I saved using Safari?

Let's say I wanted to create a simple web app to store my various usernames and passwords. The app and database would be hosted externally (by Dreamhost). I would use a secure connection for any HTTP stuff going back and forth, and I would encrypt all of the passwords before going into the db, and I can only extract and decrypt a password after entering a pass phrase that exists only in my brain. What would it take for someone else to get at my passwords?
