(1) When doing an inter-rater reliability study, does Rater A always have to be paired with Rater B, and Rater C with Rater D, etc? Or can Rater A be paired with Rater B sometimes and Rater C other times?
(2) When trying to determine the appropriate sample size for this kind of study, is the N based on the total number of subjects rated, or on the number of subjects each pair of raters rates?

Are national political polls that make use of random sample populations adjusted to account for the differences in overall population distribution throughout the US (in other words, do national political pollsters typically use some methodology to appropriately weigh responses from poll respondents depending on whether they live in more densely or less densely populated regions)?

Sometime in the last 2 years (I think!) there was a meme going around about a military theorist (from the 50's?) who came up with a rule set for fighter pilots. The idea was a fighter pilot was supposed to go around and around with these four(?) rules. Something like "assess / analyze / ?? / shoot" or something! Does anyone know who this is is, where I can get more info and the precise rule set?
