HRFilter: To whom...should I address the cover letter to?

I was taught "Dear Sir or Madam" was acceptable for cold job applications, but in my recent round of job searching, I'm seeing multiple blog posts talking about cover letters saying that this is no longer acceptable and shows a lack of initiative on the side of the job seeker. Dream Company wants me -- so why haven't I heard back from my recruiter?

Dream company headhunted me a month ago for dream job(s). They were eager to get to know me the first week, and now, four weeks later, I haven't heard from them at all despite their professed interest in me and desire to get me in for an interview. Who do I address this employment coverletter to?

I'm about to apply for a job that's a great fit for me! But one thing has me stumped. The ad says to send resumes to S_smith@perfectjob.org. The staff directory says S. Smith is the "HR assistant." However, I know the name of the executive director and the person who heads the department I would be working in. Do I address my cover letter to the HR assistant, the executive director, or the department head? (I'm pretty sure I should not direct it generically to "Hiring Manager.")
