Are cover letters dead?

I've applied to two jobs this week (the only two) for very different companies, and in both cases, when I applied on the company website, all that was requested of me was my contact information and a copy of my resume--no cover letter. [more inside]

Dream Company wants me -- so why haven't I heard back from my recruiter?

Dream company headhunted me a month ago for dream job(s). They were eager to get to know me the first week, and now, four weeks later, I haven't heard from them at all despite their professed interest in me and desire to get me in for an interview. This is weird because I am uniquely qualified for the jobs they're trying to fill and they sounded desperate. Given the communication history described behind the cut, what should my next steps be? Who do I address this employment coverletter to?

I'm about to apply for a job that's a great fit for me! But one thing has me stumped. The ad says to send resumes to S_smith@perfectjob.org. The staff directory says S. Smith is the "HR assistant." However, I know the name of the executive director and the person who heads the department I would be working in. Do I address my cover letter to the HR assistant, the executive director, or the department head? (I'm pretty sure I should not direct it generically to "Hiring Manager.")
