Advice on getting a Benefit Exception from our healthcare insurer?

Husband needs open heart surgery. Our preferred surgeon is not on our healthcare plan. We're going to request a benefit exception. We are in Washington state. What consequences will a Canadian speeding ticket have on a US resident?

I got clocked speeding in an area of Montreal with an 18 mph speed limit (that I didn't know about). I got a ticket and was sent on my way. As a US citizen and resident, I have questions about what impact (if any) this ticket will have on my license, insurance, and--if I choose not to pay the ticket--my ability to return to Canada. What is "sufficient documentation" for an ADD medication?

I've been taking Adderall XR for about a half a year, and it has made a tremendous difference in my life. The med, however, was recently put on the Prior Authorization list - which means that I now need to receive approval from my insurance before they cover it. My doc submitted an application, which I just found out was denied on the grounds that I don't have sufficient documentation of ADD. I took a test with my doc, and was denied again. Someone's car hit my mailbox and left, should I file a police report?

I came home after work today and noticed that my mailbox is tilted. Then at around 6 PM, one of my neighbors came over and told me that she witnessed a car backing up and hit my mailbox and then drove off. She was able to write down the license plate number of the vehicle for me. I'm not sure on what to do now. It's an old mail box that I've own for about 10 years and I do not mind replacing it. So I don't know if I should just go buy a new mailbox and let this incidence go. I'm a bit bothered by the fact that the driver drove off and did not accept responsibility. How should I go about handling this situation? Thanks for the suggestion!

A town-owned tree fell on my car - town says they're not responsible

A large tree across the street fell on my car while I was sleeping and did a fair amount of damage. After getting two repair estimates as requested, the town's insurance company decided they have no responsibility as they didn't have prior knowledge of any problems with the tree. I was in a fender-bender that was indisputably the other driver's fault, but I'm not sure how to proceed with the insurance claim because there was already some mild prior damage to my car from a couple of scrapes in my house's treacherous driveway that I never brought to my insurer's attention. How to get our unemployed and non-insured neighbors to pay for car damage? Our downstairs neighbors (we split a house) are borrowing an (uninsured) car from their friend over the summer. After getting super drunk early Mon, they somehow barrelled out of the garage, hitting our car and (we think) another car parked in the street. What phone/data plan is best for me?

I've long been with AT&T simply because it worked well and the cost of moving was a deterrent. After (naively) getting insurance after a stolen phone a few months ago, by latest phone was water damaged. Considering I've gone 10+ years without losing or missing a phone I never needed it insured. At the store, without looking at the contract I was told the deductible was $50, instead it is actually $200. This is enough to make me leave and pay a fee. How long after getting private health insurance can you get surgery?

I need a jaw surgery and I haven't had any insurance for the past year. I was thinking of paying for individual private insurance, but I've never had to get surgery before so I don't know if there will be restrictions to my getting surgery right after signing on to a policy. Sewage Backup. Insurance Questionable.

We had a sewage backup - we live in a townhouse, on a slab. (Two neighbors got hit, too.) My homeowners insurance covers rain, flooding, and roof problems, but not sewage. However, the cause of the backup was not our toilets - a branch dislodged on our shared roof and sent down a swimming pool full of water, which overwhelmed the sewer. Filling out an insurance form given a lot of flux

What do I put on a car insurance quote form? I am moving (NJ/NY/CA), changing jobs, and going through at least 3 unique stages of driving frequency/behavior over the next few months. And it really feels like most of this isn't the insurance company's business. But clearly I need to put *something* in each field. :) Help? Here are my facts, what parts of this should be put into the form? Work 1) I've worked full time in NY for just over a year now 2) I am unemployed between now and the end of June and for most of August 3) I have a full time, residential job in NY during July 3) I'm interviewing for jobs in CA that will begin in late August/early September and have already gotten at least one offer. But I haven't made the choice yet and won't for at least another month. Our relatively young car (~1 yr) has collected a bit of damage over the past year. One bit of damage is a dent which is starting to rust, so it needs to be dealt with in a body shop. The dent was not our fault, but the result of a parking gate that closed early. We also scratched the car elsewhere in a few places. My last job had health insurance through a certain company I'll call WORST COMPANY, and they capriciously decided to be assholes about pre-certifying a surgery that my husband needs, as per my husband's surgeon (the surgeon is ready to go, he's been treating my husband for this condition for years now, and he says this is what's required. We've done all the conservative measures and they didn't help). We were fighting with them for almost a year and it was a whole big thing including trying to appeal to the state (which didn't work because then we found out my then-employer had a self-insured plan). It's a long story but please take my word for it that it was capricious and not our fault. I just got a new job and thought "Thank god, we'll start over with someone else." Except my two options are a high-deductible plan through OKAY COMPANY and a regular PPO through...........WORST COMPANY. [more inside]

What do to after losing job in Ohio: health care, unemployement, etc..

A good friend has been fired from her low paid private school job in the Cleveland Ohio area and is looking for advice on what she needs to do in the short term. She works in a somewhat hard to find work in field (music therapy for children) She's in her mid 40s and is single, no dependents. Her health insurance will run out at the end of this month. Her salary is perhaps 30k, maybe lower. She has no savings but family in the area who will likely help w/ rent, etc.. but they are limited as well in what they can do financially to help her as they are retired on a fixed income. What are concrete steps she should take in order to make sure she has: health care (medicaid? ACA exchange insurance since she had a qualified life event?) unemployment insurance (what separation reason should she ask for for her employer NOT to fight this?) food stamps (if they exist any more for single people) links to possible sources of jobs in her field Anything else? Do I need to sign up for ACA for a few weeks to avoid penalty?

ACA penalty question. There is a 99% likelihood that I am about to be hired by a company that will provide me health coverage beginning on April 7th. I have not been given a formal offer, but I have been given verbal assurance, and I have every reason to believe I will receive an offer next week. The ACA deadline is March 31st. Do I need to sign up for ACA to cover the week between March 31 and April 7 in order to avoid paying a penalty? [more inside]
