Best way to create fantastic forms

I need to create a web form to gather information. In an ideal world, the service would create a hosted URL, people would fill out the form, when submitted, the form would automatically be emailed to 3 selected addresses, and there would be an option for the submitter to either save to pdf or print. GIF Archaeology

I'm writing a paper on (animated) GIFs and am trying to track down some of the most (in)famous. I suppose I am talking memes, but I'm more interested in the GIF as an archaeological reference point. I frequent sites like dump.fm, tumblr etc. so am quite tuned in to the glitchy/kitschy side of GIF culture. How theoretical have people got on these wonders of the web? Best designed "information" sites?

Best designed "information" sites? I've done a lot of web sites for bands / independent films / small businesses. I've now been asked to create a site for a large government organization and am scouring the web for design inspiration. What are your favorite sites (not necessarily gov't) that are clean/modern and well organized and/or offer features you think are important to a site that has a heft of wide-ranging information?
