Recently I had to spend a few hours in a psychiatric emergency waiting room where none of the staff seemed to have any consideration for the privacy of the patients being triaged there. Are any of the things I experienced violations of HIPAA or the like, and if so, what, if anything, would you do about them? (Details inside.)

An employer decides to update their employee database and distributes a form with the usual name, address, emergency contact stuff on it. There is also a line for "Any medications you are currently taking."

I am looking for advice to come up with a technological solution for a physician’s office in NB, Canada. The scenario is that I would like patient files to be accessible remotely while still satisfying health privacy laws (PIPEDA), especially the part about patient records having to be stored in Canada.
