Online Printable Legal Forms?

Is there a good reliable website that provides free printable legal forms? I am a public librarian and we have the reference books with legal forms that our users can photocopy but sometimes they just gotta use the internet and I have yet to find a good reliable site for this. For a while I had a link to a register of deeds in a county in North Carolina that had a few but they are gone now.

Is there an existing greasemonkey script, firefox plugin, etc, that will help me enter data into forms?

Is there an existing greasemonkey script, firefox plugin, etc, that will help me enter data into forms? Basically I have a web application that our company uses for bug tracking. The field where you enter most of the data is a regular textbox, that accepts HTML. Our process is to fill out a "form" and put it in this box - we have a template that we paste in and then type the data in. This is a bit tedious to say the least, error prone, etc.

I'd like to have a script that lets me "add" a button above or below the textbox, which will pop up a window with a user-defined form (checkboxes, radios, text widgets, maybe calendar widget, etc) and let me enter data that way. When I'm done it pastes this into the form in html.

I'm having trouble even figuring out good search terms for this so I come up with tons of greasemonkey scripts but it's mostly little one offs for facebook or mafiawars or something like that.

Solutions for a web-based form that takes payment...?

Have to build a form for a web site that takes generic stuff (name, address, etc.) but also processes a payment, preferably through Paypal Pro. The site is hosted by Dreamhost and the CMS is Expression Engine. Paypal Pro's site is a little mind-boggling for an amateur like myself. Are there companies that will just do this aspect of the development (and quickly) or a site that has solutions given these variables? I don't mind paying, I just need it done fast.

Workfilter: Does anyone know what a Proposed EPA Mechanism document is? I am filling out an unreasonably complicated contracting form so the small company I work for can accept contracts from government agencies. It asks us to upload various documents, but provides no explanation as to what they are or even what format to put them in.
I have been relying on Google searches to figure these out, but one eludes me: Proposed EPA Mechanism document (including supporting documentation.) Any one know what this is?

Google forms auto-notification when updated

Looking for a way to have Google forms/spreadsheet send an e-mail notification when a form has been filled out. We're thinking about using Google Forms to collect data from stakeholders. These forms would be updated at random intervals, completely unknown to those who needs the data. However, far as I can tell, there's no easy way to notify the form's owner that an update has been made. Anyone know of a way to make this happen? Bonus points for highlighting new responses.
--Yes, I know people can just manually check the spreadsheet for updates, but that doesn't always work out very well.

Editable PDFs?

I want to be able to create editable, form-based PDFs. What software do I need? I want to be able to create editable, form-based PDFs. For example, if you look at this W4 form, you can click and type information into each of the preset fields.

What software do I need to be able to do this? My google-fu is failing me; it looks like I need to buy Adobe Acrobat, but is that the only option? If it is, does it matter as far as Standard vs. Pro edition?

Please help me set up some Access forms.

I have two questions about setting up MS Access forms:
1) Currently, I have a form to enter the volunteer id, shift start time and shift stop time, but the client doesn't want to have to look up the id in a report. Is there a way to create a selection list from the volunteer's names? What other options are there?
2) On the "add/edit a volunteer" form, I'd like to be able to choose tasks from a selection list. The tasks table has id and name. The "tasks of volunteers" join table has volunteer id and task id. How do I set this up? I can't seem to get the right combination of configurations. Additional information:
1) The shifts table only has those three fields, and I don't want to make the volunteer's name a primary key.
2) I don't want to resort to adding the booleans TaskSort, TaskPrep, ..., to the volunteers table, since that's not how databases are supposed to be set up. When I try to use a select query, I get "SELECT ..." as one of my options.

* It's Access version 2003.
* I'm just using standard forms (ie. not "Pages").
* I'm pretty sure I've created the tables in a "best practices" way, but since I'm creating this database from scratch, I can create anything however I want.
* I think I know about database theory pretty well, and that's not what this question is about.
* But, I'm new to Access, so it's likely I'm missing something easy.

Thanks.

How do I electronicify my Dad's paperwork?

Help us get things electronic.

My Dad has lot of forms (an example of one here) to fill out for his little electrician business. Lots of forms for each job. How can he fill them out on the computer, save it to a database and be able to print them out? Ideally the computer version would look the same and he would be able to tab through the fields.

Any suggestions how we could do this?

Many thanks in advance
Cannot tab out of certain textboxes on the web. Help?

Why is it that certain textboxes on the web (Friendfeed, in this case) cannot be tabbed out of? In both Safari & Firefox on Mac OS X, I can use the Tab key to move the cursor into the textbox on this page, but I cannot use the Tab key to move the cursor out of the textbox.

What's the difference between that textbox and the ones on Google or Metafilter or most other sites? Why can't I tab out of it? Is there a way to fix this problem?

How do I form the First Church of [Insert Celebrity Name Here]

How do I start my own church in California? If L. Ron Hubbard can have a church, why can't I? I have tried searching for "how to start a church" on google and askmefi, but no luck. I think there's something about filing a 501c non-profit form, but there are a lot of forms for similar things (non-profit raffle? commercial coventurer?? what the crap?). This will not be a serious religious organization by any means, so any money I spend on this endeavor will be minimal. I may want to include the name of a celebrity in the name of my church (think Peter Griffin's Church of Arthur Fonzarelli). If I somehow am able to register a church, will that keep other people from using the name I pick for my church? Can I get out of paying income tax? Do priests pay tax?

It would be really nice if there was just one form I could fill out that a couple of my "congregants" could sign, get notarized and mail to the IRS or City Hall or something.

Job Center PHP or Wordpress Configuration

Looking for simple job application center online application (php is a plus and easy intergration with wordpress or plugin even better) so a client can simply post jobs and have online users fill out an application. Any suggestions? Most of the wordpress plugins I found were for Job Boards which won't work.

Using wordpress I might be able to achieve this with:

* Create a custom page template that list all items from job category.
* Use cforms II plugin to create one generic online application.
* Have HR write job description as posts to job category with "apply link" to application form in the body.

Would this work? Are there simpler methods?

How to update wordpress from outside the admin interface

Wordpress filter: I want to be able to create a form that can be used to add custom values to a certain post without having to do so from inside the admin panel. I'd also like to be able to create a form that can be used to create an entirely new post, or edit the contents of an existing post.

Sample code to work from would be excellent. Directly pointing to resources that explain the inner workings of Wordpress may be less excellent, as I have only a rudimentary grasp of basic PHP and would likely be over my head.

Alternatives to OmniForm?

I'm wondering what alternatives to Nuance OmniForm are out there. They haven't updated the software in many years. The "standard version" costs $99 and the "premium version" costs $499. Holy crap! Basically I'm interested in being able to pretty easily create electronic forms that are the digital equivalent to paperwork that can be printed, emailed, faxed, etc. Some of them might contain simple math (like on an invoice).

My problems with OmniForm are that the layout tools, interface, and documentation are really poor. It can be tough for example to get various items aligned properly.

I'm interested in alternatives for both Mac and Windows. Free, as always is best, but around the $100 mark would be ok as well.

From the limited results that google searching has found for me, it looks like I can somehow do this with a PDF?

How to make multiselect dropdown lists

Form creation on MS Word 2003 on an XP system. How do I make an MS Word doc with a multiple-selection dropdown listbox? I know this can be done using VB, but I don't know VB. I've made forms before using dropdown boxes from the Forms toolbar in Word (as opposed to the Control Toolbox toolbar) but the user can make only one selection from the list.

I'd like the dropdown list to allow for non-contiguous item selection. That is, if the list items were "red", "green", "yellow", "blue", "orange", and "purple", a user could select one, any, or all of them.

I need one of the following:

a) clear instructions to use the built-in tools in Word to do what I want
b) a reference that a complete VB noob can use to build a multi-select dropdown listbox -- fast.

Can anyone help with one or the other?

Recurring text box in Word?

I need to make a sticker sheet in MS Word updatable -- all the stickers on the sheet look the same, but if I type "Q" in the first sticker, "Q" will appear on all the other stickers. Is this possible? Working on making an Avery-compatible label sheet for a company that only sells eight different things, but in a range of varying sizes. So ideally, he'll be able to print label sheets one sheet at a time, with the same product type on every label on the sheet (say, GRAPPLEGROMMET ALPHA), and then a space to the left of the first GRAPPLEGROMMET ALPHA sticker, where he can type an XXS, XS, S, M, L, XL or XXL and see every other sticker on the sheet update automatically.

Idea being this saves (a) the time-consuming process of typing S on every single sticker on the sheet when he needs a bunch of "S" stickers for GRAPPLEGROMMET ALPHA, and (b) saves the time-consuming process of doing that ourselves, and then creating 64 label templates when all we really need are 8.

I would think that there's a simple way of doing this, but everything I look at veers abruptly into Visual Basic programming and odd form-driven functions that don't really work. Is there some way of just making a text box whose contents will be copied in linked text boxes across the same document?

need quick-and-dirty html help :o)

I have only the most basic HTML skills and I am posting a form on my Typepad blog, with some HTML provided by Yahoo groups. It all works great, except I would like to center the form on the page. I am hoping that someone here in the "hive mind" would be kind enough to tell me what code to put where in the following html so that the form will center? Many thanks!!! (code below) Here is the code (with all identifying information removed and every <>-style bracket replaced with square brackets so it would display here).

[form method="get"
action="http://groups.yahoo.com/subscribe/xyxyxyx"
style="background-color: #ffc; width: 254px; padding: 2px 4px"]
[h6 style="font-weight: bold; margin-bottom: 5px; text-align: center"]
Have Film. Need Release Forms

Where can I find a basic "release" form on-line? I work at local farmer's markets selling raw dairy. Over the next couple of weeks, I am going to film customers telling their positive experiences with our product. These bits are going to be 2-3 minutes long and will be posted on a website early next year. The organization operating this website will be non-profit

I realize that I need to have them sign a release form authorizing us to use their images and words on the website. Is there a basic release form that I can find on line?

I have a friend who has a small business and wants me to perform a LAN/WAN Network Assessment. It's something I never done before but I'm fully capable. Instead of me reinventing the wheel does anyone have fill in the blank forms or links to forms with questions I should ask and what I should inventory? Thanks!

I need some examples of short employee self-evaluation forms. Our small non-profit is currently using one that is seven pages long. When I asked if we could shorten it, they put me in charge of coming up with a new one. Sigh.

Convert printable booking form to emailable?

I am helping out with a small community event - I have been given a booking form formatted for printing (a word document).

What is the easiest and most accessible variety of ways for me to convert this, so that I can email it out to everyone, and have people fill it in and email it back to me? Users, are not especially computer literate and I'm doing this for free.
I'm trying to figure out the best compromise.

I can send it out in more than one format for accessibility/ease of use for the most number of people. Last year several people just tried to edit the word document, but couldn't edit check boxes etc.

Send that email, stat !

I'm working for a newer growing, small business (4-5 employees or so).
We have to send emails containing the same information to existing customers.
For example, we have to notify customers about their accounts (for
example, their CC/billing information becomes outdated) or customers
send us a question. We're looking for an efficient way to do this. Right now, we want to send out emails more efficient and rich than our current system:
using Apple Mail (our office is using Mac OS X Tiger, and will
continue to use Tiger, and Leopard, once Tiger is no longer supported
by Apple) having a templates folder, selecting the send again button and then inserting the person's email. More importantly, Apple Mail (to the best of my knowledge) doesn't send out emails in HTML. Also, the ability to edit these emails before we send them out (like, adding an extra sentence to it before hitting send).

Instead, my boss is looking for a system that would create more professional looking emails (ones that you get from an established web business) and will allow someone with no web development expertise to send them out to customers.

Related to this, is there a way to automate this so that an html version of the email, and a plain text version is also automatically generated ?
(For this second question, I'm hoping there's an easier way
than writing a script from scratch (I'm familiar with java, but I think that isn't appropriate for this) to assign the content that changes to variables, and then inserts the content to the variables in templates that already contain the formatting.)

Forms look like a way to go, but I'm not sure if javascript or PHP is a better way. I'm leaning towards php, because I read there's the ability to integrate php with sql, eventually creating a database record of customer interaction (phone calls, emails, etc).

Is this possible? Begging for some type of assistance!

Joomla 1.5 form puzzle. Looking for a way (either a plug-in or module) to let me create a custom form that will allow users to input a code and based on the day the code is inputted, assign a numeric value... Ok, so I have a bit of a conundrum, I have a site, built with Joomla 1.5 and I want to make a page where users can input receipt numbers for various things. Say you received a receipt from a store, and what we want to try and accomplish (as none of us are really certain if this is doable) is the customer would input this receipt, and based on the day the receipt is inputted, there would be a set assigned variable...

For instance, say the users receipt number is 77804 and he put this number into the form on a Monday. If you put a receipt in on a Monday, the receipt value would be 3, if it were a Friday it would be a 7...etc etc....

Is this possible using Joomla and if so, how can we accomplish this? I know there are dozens of form plugins for Joomla, we just don't know what ones would work the best and which ones can actually do this.

Finally, after the user inputs the receipts, we as the "end users" would get an email or something accessible where only the admins can see the results of the receipt totals.

Looking for any info on this as its something we have been trying to figure out for almost 2 months now and we are just not getting any where. Please, MiFi super geeks, nerds, Joomla advocates and designers, we are begging to find a way to make this work. We might even be able to give some type of trade or something for any help with this question....do I sound desperate enough!?

Can I write to a div using Perl, or otherwise solve my form submission problem?

Can I write to a div using Perl, or otherwise solve my form submission problem? I have a Perl/CGI form mailer script that I didn't write and shouldn't edit too much, but can somewhat.

It's currently set up so that I can define the thank you page and error page within the Perl file. I don't want it to redirect to those pages however. I want to display the messages on the page the form is on instead, without reloading if possible. I played around with redirecting to the same page but appending a parameter in the URL. Then using JS to show a div if that var was set. But I'm realizing I need to display some of the items submitted from the form and I don't know how to keep those values since the page is being refreshed.

I don't know Perl so I don't know if there is a better way to do this, but can I do something like write to a div with Perl? Other ideas?

Does there exist a lightweight and attractive forms based database?

Can you recommend a forms based database like Filemaker, MS Access, or OoO Base, but lightweight? I'm using Base right now and it works well, but it's a huge mammoth of a thing and takes up a lot of memory and CPU that I'd rather use for other things. (I use it on a laptop when I DJ so taking over the CPU mid show is a problem.) I'm looking for something that is, in order of importance:

- Windows (XP)
- Not a resource hog
- Not web based.
- Has highly customizable forms.
- Has decent data export (csv, excel, etc)

It doesn't need to deal with millions of records so a SQLite based DB would be enough. Price is less of a consideration that the rest.

It's been forever since I've used FileMaker (Version 5 I think), but I remember it being a similar beast. Is that still true?
