What do I ask/tell my new doctor about my mental health?

I don't know what help I need anymore, or even if I need help. I have a lifelong history of depression and the past two and half years, I've experienced more life stressors than normal. The original reasons I made the appointment was to a. get a new script for my anti-anxiety medication (which is also meant to help my depression), especially as I accidentally abruptly ceased it over 5 days ago, and b. a referral to a new psychiatrist to prescribe ADHD medication. Is a referral better than a cold call? How do they work?

My insurance doesn't require referrals to specialists. I've never seen a specialist for a specific problem that wasn't initiated and referred by my doctor. If I want/need to see a specialist for something, does it make sense to see my doctor first and ask for a referral, or find a (random) specialist myself? On the other side, when does a doctor not give referrals? [more inside]

House says it's not Lupus, but I disagree.

I have many of the symptoms of Lupus, and my mother has the genetic defects that cause a number of autoimmune diseases including Celiac and Lupus. I'm generally a pretty nervous person. Lately (over the last two months), it's become worse. I jump whenever someone enters the room.
