There was a fraudulent online charge on my debit card which I'm dealing with disputing with my bank. What passwords should I be changing or other tactics now to protect myself and the new card I'll be getting soon?

Today, my bank's fraud detection department gave me a call - apparently, somebody had attempted to use my debit card for a purchase in Colombia. I live in London, UK. This is an annoyance, but it's particularly worrying because something similar happened less than two months ago - they called me up and said that although my card hadn't been used, they had information that it may be compromised. The card that was used in Colombia today was the replacement, which I've had for only a few weeks.
My money is safe and my card has been cancelled (again). But my main concern is HOW this has happened - keylogger? Dubious, but legit-seeming site? Compromised wifi? And how do I keep myself safe in future? [more inside]

I need a card I can charge with money, online, for someone else's use in grocery stores, department stores, gas stations, etc. They don't need to be able to get cash with it. It shouldn't be possible for them to take out more money than I put in. This will be an ongoing thing, so I'd like to minimize fees. Has anyone had any experience with something like this that they liked?

I need a bank account for some work-related stuff. I need a debit card for it, like yesterday. What nationally available banks can I open a checking account with that will give me a (temporary, at least) debit card immediately? Bonus if they have a lower than $20 minimum balance. I'll be arriving in Wharton, NJ tomorrow to open this, so bigger bonus if there's a branch there.

I was recently in Canada (from US) and tried to use my debit card to buy some groceries. Two different cards were rejected, for different reason codes. I would have experimented more, but I was standing between a rapidly angering Canadian woman and her beer. I see by Googling that Canadian debit cards use a different network, Interac. Are there any banks/credit unions that issue debit cards usable in both Canada and USA. I know I could've used ATMs and cash, but this would make for a less hassling cross-border experience.

When I pay by credit card or debit card in most stores (NYC), the entire process is automated except for one step. The clerk always has to ask me "Debit or credit?" The card machine also asks me that -- in fact it forces me to choose a payment type -- but my choice doesn't do anything (apparently). I choose debit
and checking account, enter my pin number, decline cash back, accept the amount... and then the clerk asks, "Debit or credit?"
And when I say "Debit," he pushes a button on his side of the counter. Why?
