I've been offered a new contract/consulting job where I would be paid not by the hour or for a specific deliverable, but at a flat weekly rate. Under this arrangement, how might I make intellectual property distinctions between works that will belong to this client, and outside works I may do for myself or others? [more inside]

Today, my employer game a form to fill in regarding my health. On it were the usual questions about diabetes and epilepsy and such. So you're self-employed, and you sign a contract for a moderate-sized, long-term gig (worth $5,000 or so/year), and...you lose your copy of the contract. Your relations with this company over the years have been overwhelmingly friendly and positive. What do you do? Just ask them nicely for a copy?
