Looking for help with divorce paperwork in LA.

I haven't seen my wife in almost two years. We decided to separate in late 2011 after 5 years of marriage. It was totally amicable, we got married too young, etc etc. For many reasons, neither of us pursued a legal divorce. Largely it was a matter of cost- She's a PhD student, I'm a Grad student, neither of us had the $325 filing fee. We fell out of touch and it kind of kicked around the back of my head. This year, taxes rolled around and we realized that we needed to pursue the legal divorce. And then we sat on it some more until I finally got one of those DIY Divorce Books. So, great, I got all the forms from the state, filled them out, scraped together the filing fee and then froze. [more inside]

I have insurance goddamn it!

My doctor's office billed me up front for an IUD and told me I would be reimbursed. Today, after calling them repeatedly, I found out that they have not filed a claim for the device, and they couldn't give me a satisfactory explanation as to why. I have no idea what to do now. [more inside]

"There was a crooked man, who walked a crooked mile. He had a crooked cane, and he wore a crooked smile."

Due to a recent and very vigorous visit to pukesville, I'm pretty sure that I've dislocated my jaw. Would a walk-in clinic or urgent care clinic (if those exist) be able to put my jawbones back in order, or does this mean a lengthy trip to the ER? I am in downtown Toronto. [more inside]

Adult beginner soccer training/camps in the U.S.?

Adult beginner soccer training/camps in the U.S.? I'm a relatively new player (started a couple of years ago). I play on a couple of teams, but as fun as games are they're just not the best way to develop basic skills; I really need a structured practice environment and feedback. I'm considering going to this camp, but I'm wondering if there's anything similar in the U.S. (Google turns up a bunch of camps for kids, but very few for adults, and even fewer for adult beginners.) West Coast would be best, but I'm willing (obviously) to travel further afield if need be.

toronto options for weekend bursitis treatment

What options do I have for what seems like a sudden acute bursitis attack in my right shoulder? I'm in Toronto and have an appointment for my first cortisone shot Tuesday (I may be able to successfully beg a shot on Monday), but I don't know if I can wait that long for my clinic to open again. It hurts a lot and I can't even sleep through the night. It's been getting worse since waking up Friday morning, not better. [more inside]

Gyn walk-in clinic in NYC or nearby?

Does anyone know of a clinic with a gyn doctor where my gf can get an appointment or walk-in today (Saturday)? We're currently in Manhattan, but have a car so we can travel to any borough, NJ, etc. If this information is relevant: The issue is that "something doesn't feel right," e.g., pain during penetration.
