Very late W-2

My employer has not given me my W-2. I've contacted the IRS, and gotten a substitute form that I have to fill out myself, but since I was never given paystubs, (tax information was usually written on the paycheck itself, and, being silly.lazy, I didn't photocopy my checks,) I have to guess at the amount I owe. I'm also concerned about filing for Arizona income tax, but when I called them they said I don't need a W-2, which dosen't make much sense. I've gotten the run-around from my employer (the company owner,) with her blaming the accountant. I want my W-2, but I also think they deserve some sort of punishment/sanction for this. What can I do?

I have been freelancing as TV producer for one company for over 6 months now working on various projects. The boss thinks he needs me to stand down for a few weeks to keep my independent contractor status. All of the IRS info I have found is vague on the point of continuous employment. How long will I have to stand down (aka take the vacation I want to take) before I can return to this company? H&R Block sells kinda pricey tax preparation software and for an extra $15 bucks, or so, you can file online using that software. Or, you can go to the IRS website and grab a link that allows you to prepare AND FILE your taxes for FREE -- using H&R Block's website. This makes no sense to me. Why on earth would they do that???
