Pregnant in New York: foreign, partly insured, need prenatal care help

My unmarried partner and I are pregnant for the first time, while on a six-month working trip to NYC (four months left). We found out on the weekend and in order to be (partly) covered by our insurance for prenatal care, we need to provide a LOT of information to the insurance company: chosen providers, required tests, costs, etc. We don’t have a clue where to start with this: not just because this is our first pregnancy, but also because the American health care system is so foreign to us. Do you have experience with a home birthing tub?

My wife is having a home birth with a water tub and midwife. The 2nd floor in our home makes the most sense for the tub in order to have access to a bedroom and other comforts. I am concerned about the weight. I have calculated the volume of water and the weight is around 3000 lbs. Is that too much weight for the floor? Have you had a birthing tub upstairs in your home? Any engineers want to chime in?

My brother and his fiancee are discussing having a tropical destination wedding next winter, where the family could buy a resort package. Due to my history of pulmonary embolism, I have been told I need a high-risk obstetrician to handle my prenatal care and delivery. However, I want the more hand-holdy and actually present approach of a midwife. But you have to choose one or the other. Is there a team-based practice or some sort that I can use? I am scheduled to take a very long flight to go see a dear old friend in her new city in a few weeks. But it turn out I'm pregnant (oops! But yay!).
