if you get a cut on your face that requires stitches and you go to the ER is it better to ask for a plastic surgeon or a maxillofacial to stitch you up? And is the hospital required to give you one if you ask for it?

I'm almost certain I just severed the tendon in my pinky...the bleeding is under control but I cannot bend it. I have severe fear of ER costs due to bad past experiences that cost me lots and lots of money. How bad is this, and can it wait for an Urgent Care place?

I just returned from a month in a country where Typhoid is common, and I have been feeling really bad for these couple of weeks now, with a sore throat, cough, feverishness, and now lethargy/feelings of severe depression/delirium/confusion for a few days now. Now it looks like I have just developed a rash on my stomach that looks similar to the less-dire images I've seen online. It's been my joke-y diagnosis up until now, but does this rash mean I should go to the ER, or wait until tomorrow and try to see my doctor?

A month ago I went to the hospital ER (tiny city) as it was Saturday and I had some weird chest pain. Basically the doctor asked a few questions, an EKG, blood test, and chest X-ray was done, and I was out in 45 minutes after observation. The damage: $475 for the doctor, $95 for the blood test, and $1800 for the hospital (unspecified). No insurance. I'm glad to pay for my care, but this is ridiculous. Any good tips for bargaining this down? Obviously I am in the U.S.
