What do I ask/tell my new doctor about my mental health?

I don't know what help I need anymore, or even if I need help. I have a lifelong history of depression and the past two and half years, I've experienced more life stressors than normal. The original reasons I made the appointment was to a. get a new script for my anti-anxiety medication (which is also meant to help my depression), especially as I accidentally abruptly ceased it over 5 days ago, and b. a referral to a new psychiatrist to prescribe ADHD medication. Need a recommendation for an ADHD doc in Toronto

I need diagnosis and treatment. Looking for a good ADHD doc/clinic in Toronto, someone who can offer or at least refer me to psychotherapy as well as prescribing medication. Preferably someone who would take a referral from my family doc and will take ohip/insurance, but I'm willing to pay out of pocket if necessary. I'm located in the east end, someone on a subway line is preferable.

I've decided to actually use my medical insurance as it was intended, rather than waiting for some body part to fall off. I have my first ever physical scheduled in 2 weeks. How does one go about receiving an ADD diagnosis and medical treatment these days?

I'm a 28 year old male in Las Vegas, Nevada. When I was in my late pre-teens, I was diagnosed with Attention Deficit Disorder, and went through various treatments (Ritalin, Adderall, then eventually easing off medications entirely). Now, due to a promising promotion and other life changes, I need to be able to focus beyond what my self-regulation allows me. Doctor won't perscribe anything for ADD

I don't have a history of ADD but things became fairly complicated lately and I'm certain after doing hours of research that that I have some form of adult ADD. My doctor is refusing to prescribe anything and believes the only reason I am stressed is because of work/school/family. He suggested taking vitamins and getting more sleep. What gives? What are my options besides switching to a different doctor?
