		<description>Medical wallet card summary thing - is there one?  &lt;br /&gt;&lt;br /&gt; Hi folks,&lt;br&gt;

Looking for a handy medical wallet card summary thingy for a family member with a lot of medications - something small enough to fit into a purse (doesn&apos;t have to be wallet sized). I&apos;m aware of the &lt;a href=&quot;http://shop.getbuttonedup.com/index.asp?PageAction=VIEWPROD&amp;ProdID=8&quot;&gt;Pocket Doc &lt;/a&gt;, which is great but doesn&apos;t have much space to write lots of medications down, and I want to put a list of her doctors as well.&lt;br&gt;
&lt;br&gt;
Anyone aware of a product I can buy, or a template online that I can fill out I can whip one up in Illustrator?&lt;br&gt;

		<description>I summarized my med info when I took a recent trip out of the country by simply doing it in Word in two columns so it was on one sheet of paper. And I just put it with my passport. BTW, I decided to include all vitamins and supplements seeing how they can affect prescription interactions.</description>

		<description>My partner takes over a dozen medications daily.  I keep a list in a spreadsheet, and change it as needed when the doctors change his meds.  Basically, it&apos;s just one column each for medication name, dosage, and how many times a day (or other comment).  When I print it out, it&apos;s still less than a single sheet and is easily foldable.&lt;br&gt;
&lt;br&gt;
There&apos;s additional worksheets in the same file listing all his doctors and contact info and also all his past surgeries.</description>

		<description>Make your own in Word, and take it to a copy shop for them to laminate.  Cheap, easy, and has exactly what you need (and want) on the card.</description>

		<description>i use index cards. keep emergency contact info, dr&apos;s name &amp;amp; number, medications, health insurance company &amp;amp; number, and maybe a few other things i&apos;m forgetting. i have one in my car, one in my house by the door, and one at work. i&apos;d not seen the pocket docs &amp;amp; might actually pick up one, but i can see what you mean about it not having much space to write.</description>
