How do I create offline surveys?

What's the best way to create a survey for offline use? I want people to fill out a survey to collect suggestions/requirements for a web site redesign. It's at an event in which there is no Internet access, so I thought there may be a way to bring a laptop and store the results (and create reports) locally.

I want something that's easy to create surveys (e.g., Zoomerang or Survey Monkey; Access form creation=yuck) and I'm assuming that any solution would have a web front end and use mySQL as a repository. I'm on Windows XP, BTW.

I've used ClassApps when we needed something a step above Survey Monkey. It's ASP (or .NET) that runs on IIS using Access or SQL server. IIS will run on XP, but it has a connection limit (5?).
