What does the asterisk mean on a US visa?

My friend has a visa to travel to the US. Underneath his photo is one asterisk. I understand there can be from 0 to 3 of them there. What do they mean? Also, under Visa type/class it says R B1/B2. Does this mean regular business/pleasure, or are the R and B parts separate fields?

I have three asterisks. I think they are there because my passport was stolen when I was overseas. I have never had any problems at airports because of it.

R B1/B2 is just a normal business and/or pleasure Visa.

No idea about the asterisks. I stuck a question up on the wikipedia talk page for US Visas though. Might get some answeres there eventually.
