Custom Form

FormFilter: I need a form that i can use and put custom fields/entries in there that i can check off and add notes to then automatically email the completed form out, preferably something in word or excel. I cant program at all so is there anywhere i can get what im looking for?

Isn't that what Microsoft Access is for? I think it comes in the MS Office suite with Excel and Word.

You can also do it in Excel and Word, it just requires some elbow grease.

I personally prefer Acrobat for building forms these days, as so many people have so many different versions of Office, and the Acrobat Reader is free. On the plus side, it's pretty easy to build things that your office monkeys use every day, like Fax cover sheets, HR forms, etc., and then make them read only except for the form data you want them to fill in. If you're feeling brave, you can control the method and nature of entry through Javascript.

I second Acrobat. The latest version (8) will even collect the data from emailed forms.

Yes, Acrobat rocks, but it's pricey and you might not have that option.

Can you create a form (using the forms toolbar) in Word and then email it using mail merge or just saving the file and mail it? That's what I do for work because people just don't 'get' Acrobat for some reason here.
