Like intentional or accidental? These short questions leave so much room for interpretation.

An intentional one I just thought of: Follow someone into a store... as they pass through the door, speed up behind them. As the door closes in their wake, slam the door with the palm of your hand, then pretend the door swung back and hit you.

For added effect you could glue a smashed cigarette to your face.

In any situation or do you have something particular in mind?

I had an ex-girlfriend who, when someone said something rude to her, would adopt a shaky voice and tell them she was sorry but her father just died. It would catch them completely off guard and the reaction was priceless.
