Short-run-plastic-fab-filter: Where can I go to order a very short run (100-300 items) of custom shaped and machined plastic sheets? In addition to a whacky shape, I need some drilling and something close to 'etching' on the surface.

Relevant details:
* I'd prefer to do this locally (seattle) if possible but I'm OK doing this by mail if necessary.
* I'm basically putting together an award for a small group of people.
* Chances are good that I'd want to do the same again in a year or two
* The goal of the etching is to permanently print or stencil the surface of the (Translucent) plastic with a design and some words
* Also, how could I do the same with Glass?

By "shaped" do you meant formed in three dimensions, or cut into a flat shape? There will be plenty of punching shops in your area, but the etching is more involved and expensive. Look for "laser marking" or "laser etching".

google gives me these two as likely candidates.
