Quickbooks Pro Export Question?

Does anyone know how to get QuickBooks-pro to spit out a form that looks like this? Or has all the elements of this? Caltrans doesn’t care what order, it all just needs to be there. Is there an add-on to QuickBooks for Certified Payroll. (Already tried Sunburst: It sucks). Bonus points for low cost solution (blew my wad on the Sunburst fiasco.)

My version of QBP is now quite old, but the forms desginer never did anything for me (couldn't seem to even go to landscape). I would think the easiest way is to find a way to get the data into something you already know for the formatting, like Excel or Access.

I looked around real quick, thought these might be worth a try:
AccessBooks RT ($199)
AccessBooks ($99)
ExcelBooks ($176)

I haven't used them, they just seem like places I'd start (probably with the cheapest way just to get the tabular data) if I had to do this.
