Can you recommend a good wordpress theme (or other tool) to build a personal blog that contains really long form writing: book chapters, or long academic articles?
I like Medium's style but it is not all about the look.
Table of content is a must.
I also want to know if there is any good way to get reader's feedback by letting them annotate and commenting on particular lines...
or is there any tool that can help people split long stuff into different parts and then automatically link them together?

Check out the Origami theme by Site Origin. I doesn't do everything you want, but it's minimal and content-focused. Plug-ins may fill in the gaps.
