Adobe Acrobat X Pro Forms

I need to get Adobe Acrobat X Pro import text data into a form (this part is not an issue) and then get a bunch of PDFs with the text data in the proper fields. I've done this with no plugins/scripting/etc. whatsoever before and just can't remember how.

EXTREMELY IMPORTANT NOTE: I am 105% sure I have done this with nothing but vanilla Adobe Acrobat X Pro and no scripting whatsoever. I have all the files to prove it. I just don't remember the process and Google is throwing up nothing but plugins and scripting solutions that, again, I know for a fact I did not use last time. I also didn't use Actions or Mail Merge in Word or any sort of Acrobat/Word functionality.

I'm able to import the data in Acrobat but I can only seem to do it manually line-by-line. I manage to automate this last time but I don't remember if it spat out a bunch of individual files or one long file (like how Word does with Mail Merges.)

Help.
