Help me remember the name of this movie.....

I'm looking for a movie title. It is subtitled and set in Italy post WWII. The premise is about a young pre-pubescent boy (he narrates the movie) and the trials he faces with living in a small, over-crowded apt. He is becoming more and more of a man throughout the movie. He can't escape relatives crowding the apt to facilitate this change/behavior. The opening scene is an explanation of the bizarre way he was conceived. Something about a tomato cart, his mother falling into it, and her not wearing underwear that day? The young man's name starts with an L. His name is the title of the movie. There is a scene where his mother has asked him to pick up liver from the butcher. Before delivering it to her for dinner preparations, he slices into it with a knife and uses it to mimic a woman's vagina. I hope this is enough information. Thanks.
