Fixed price taxi app in San Francisco?

Uber is very handy, but my pet peeve is that you can't get a price quote in advance. In fact, during times of "peak demand," some folks have really been soaked by Uber. I want something like lecab.fr in Paris, but for San Francisco. I input origin and destination and get a firm quote. Bonus points for being able to schedule that for a specific time, inclusion of gratuity, and the ability to see the driver assigned to me as she is on route.

It's not technically a "taxi" but Lyft and Sidecar show you the "suggested amount" before you confirm your ride.

Taxi Magic uses "real" taxis and you see the estimated amount, but since it's SF it's dicey if they will actually show up.

You can see an estimate in Uber. There is a button on the page that shows which credit card you will use to estimate the price. It isn't guaranteed, so not truly flat-rate, but it is is possible.
