Google App Script - no error generated?

Google App Script - no error generated? I have two scripts that run on form submit, but sometimes they don't run at all. No emailed errors, no messages to the execution transcript or log.
The scripts generate emails to multiple people and email's a pdf document (kind of a mail merge) to a few folks. I have had the spreadsheet open in a browser window when someone else submitted and it didn't run, so maybe that was it. But when I test with the spreadsheet open, the scripts run. The two scripts I'm using are based on the code here and here.
