What is this poetic form called?

There are six stanzas. The first five consist of three lines. The final stanza has four lines. The first and third lines of each stanza rhyme while the second line of all the other stanzas rhyme with each other. The first and third lines of the first stanza repeat as the last lines of the remaining stanzas alternating with each stanza until the final stanza where they are the last two lines.

I'm not sure if there are other rules, such as syllable count.
