HTML Form Generation Questions

HTML FormsFilter: I'm building a short multiple choice survey. Is there a good, free online application which will help me do this?

Hand coding HTML forms is a real pain. I'd prefer something that gives me CSS styling too. I'm sure I reember see this but can't find it anywhere. Thanks!

BTW I'd liek to do my own hosting - I just need the form created - not the backend.

Nvu is not an online app, but it is cross-platform and contains a WYSIWYG HTML editor, which will let you create forms and CSS files easily. Copy-and-paste will let you automate much of the work.

Brilliant - thanks for all the help. Form Builder was exactly the app I was thinking of.

I also downloaded Nvu and it seems to be a good lightweight alternative to Dreamweaver.
