Help me find simplified/well-designed versions of useful/common forms.
A while back I ran across a collection of useful/common forms that had been redesigned to be simpler and easier to read. I think maybe one was related to medicine--a follow-up after a physical?--and there were some other personal finance ones. This simplified credit card agreement is the right idea, but thought I remembered seeing more of them. I'm trying to find them now, but my Googling is drawing a complete blank.
