I can't sleep. Also, I can't wake up!

I think I have delayed sleep phase disorder. Recently I found an online questionnaire that asked questions about your natural preferred wake and sleep times, along with the times necessary to fit into your actual life schedule. It would then calculate what time you should expose yourself to very bright light, and for what periods of time in order to shift your wake and sleep patterns. I cannot find it now that I really need to change my schedule. Anyone have any idea where I could find this site again?
