Online form-to-text-document application?

Is there a free online service that allows me to create a form that, when the fields are filled in, generates a text document with both questions and answers that can be printed out?

Depending on how much work you're willing to do, you may be able to create a form and try to mail merge into a Google Doc.

Zoho appears to have 'proper' mail merge but you may have to pay for that (I've never used it).

Another option may be Wufoo, or a similar service, but it looks like the export is only to Excel/CSV.

SurveyMonkey lets you create pdfs. A basic membership is free.
