Who comes up with the exotic activities and wacky, contrived shenanigans on travel shows? The writers? The host? I bet that'd be the greatest job ever.

The show's producers and writers.

Tony Bourdain's Cook's Tour book has some info on this.

On Bourdain's shows he often talks about the 'fixers', the locals who will take him through market or get their mother to cook for him. Often locations are presented as the fixer's idea, although part of that could just be television reality (like the 'stunt fish').

I Googled 'fixer' and found Tokyo Fixer, which sounds like a great example of this.
