Dress me up, dress me down

New York-area dressmaker and/or designer recommendations, please.
I need a formal gown. Dressmaker, designer, whatever - I just need help and guidance. I have ideas about silhouette, fabric and style, but they're vague enough for me to want a dressmaker with a really great eye, a stylebook, and a sharp, opinionated tongue.
Budget-ish is better than high-end, five boroughs better than the suburbs, but all recommendations gratefully taken.

I don't know how expensive their work is but it's phenomenal. They seem really attentive to clients. It'd make a fun trip to Philly!
