Where can I find a basic "release" form on-line?

I work at local farmer's markets selling raw dairy. Over the next couple of weeks, I am going to film customers telling their positive experiences with our product. These bits are going to be 2-3 minutes long and will be posted on a website early next year. The organization operating this website will be non-profit

I realize that I need to have them sign a release form authorizing us to use their images and words on the website. Is there a basic release form that I can find on line?

A google search for "model release" will turn up quite a few options... Your safest option is going to be to have a release created (with appropriate legal consultation) specifically for you though.

I usually check out docstoc.com for this kind of thing. Here's an appearance release that you may be able to use, and there are several related documents that might fit the bill as well.

Forgot to add that you might want to get a location release from the market if you're going to be filming there.

IANAL, but I have worked with these types of releases before. The doctstoc.com release looks pretty much like what I have used in the past. Note that with that document you would only be able to use the footage for that particular project or the promotion of that project. If you are a little iffy on the exact specific use, you may want to make the top section a little more generic (not about a specific project, maybe generalize it to be used to promote the local market in general).

The document also mentions "good and valuable consideration", so the person you are filming should get something. It could be as small a free sample of the product.

I repeat though, IANAL.

If you have any lawyer friends and can leverage the charity aspect of this, they would likely be able to read over the stock document and make any changes as a favour, it's a really basic job. Most lawyers would probably have a similar stock document they provide their clients.
